Your assessment of needs is commenced prior to your admission to the nursing home by a pre admission visit by the Director of Nursing at your home or hospital as appropriate to establish that we can meet your individual needs and expectations of care.
A comprehensive care plan is developed based on your assessment following admission to set out your physical, personal, psychological, social, medical, religious and cultural and any preferences you may have regarding your daily life.
Your care plans will be reviewed with you and your family as appropriate on a regular basis or as your care needs change.
